N-648

The N-648 form, or Medical Certification for Disability Exceptions, is used to request an exception to the English and U.S. History and Civics testing requirements for U.S. citizenship (naturalization) due to a physical or developmental disability or mental impairment. For the N-648 a neuropsychological evaluation is conducted, including the assessment of mood, symptoms and functioning, as well as cognitive skills such as memory, concentration, orientation and executive functioning to determine a diagnosis, assess the applicant's condition and explain how it prevents them from meeting the testing requirements.
